How/When/Where to Refer

Referral completed by Primary Care Provider (PCP) and sent to Central Intake (CI) by fax (613-721-7889) or by direct referral through the Ocean eReferral system.  This applies to the following conditions:

Hip/Knee Osteoarthritis for potential replacement referral form:  Patients with moderate to severe osteoarthritis should be referred for advice and potential surgical intervention.

Foot and Ankle Conditions referral form: Due to long wait times for surgical consult, PCP’s should direct patients to try conservative measures while waiting.

Shoulder Conditions referral form: Due to long wait times for surgical consult, PCP’s should direct patients to try conservative measures while waiting. A referral to a sports medicine physician may also be considered for these patients.

Knee Arthroscopy referral form: Due to long wait times for surgical consult, PCP’s should direct patients to try conservative measures while waiting. A referral to a sports medicine physician may also be considered for these patients.

Non-ISAEC spine referral form: Due to long wait times for surgical consult, PCP’s should direct patients to try conservative measures while waiting.

ISAEC (also known as Low Back Pain Rapid Access Clinic):

Interprofessional Spine Assessment & Education Clinic (ISAEC) only accepts eReferral. Referral completed by PCP and sent to ISAEC Central Intake via eReferral on LHINworks or Ocean for integrated electronic medical records (EMRs) (Telus, OSCAR, Accuro).

The program is appropriate for all patients with more than 6 weeks to less than 12 months low back or low back related leg pain e.g. sciatica.

This includes any patients you feel may warrant further diagnostic imaging or surgery for lumbar spine conditions as the ISAEC model expedites these services.

You may send patients with chronic symptoms only if they have had an acute flare up within the last 12 months. They will only be seen for management of the acute flare up in this instance.

eReferral

eReferral is now available for all primary care providers in the Champlain region. 

Ocean eReferral can be installed directly in your EMR for users of Telus PSS, Accuro and certain versions of OSCAR. This eReferral solution comes at no cost to referring physicians and our team can assist you with the installation.

Ocean eReferral will allow you to send your Hip & Knee, MSK and ISAEC(1), (2) referrals directly from the patient’s chart. The referral form will auto-populate with the patient’s information and a copy of the referral will automatically be added to the chart.

(1) Please note that providers must have completed registration and a brief on-line training in order to be able to refer to ISAEC
(2) If you cannot use Ocean eReferral, ISAEC referrals are still required to be submitted electronically, leveraging the existing eConsult platform (Healthcareworks) in the Champlain region.
